Dr. Bismah Irfan, MD is a board certified nephrologist who sees patients in person at Kidney Holistic Institute in Westchase, Houston, and by video across Texas and 13 other states. The first appointment is spent on the question most kidney patients have never been asked: what started the damage, and how much of it can still be moved.
Houston has one of the largest kidney patient populations in the country and one of the highest rates of diabetes and high blood pressure behind it. Most people who come to us have been followed for years, sometimes inside the Texas Medical Center, and have watched their numbers slide while being told the plan is "working." The numbers were being managed. The causes were not being looked for.
That is the gap this practice fills. Dr. Bismah Irfan trained and taught as a conventional nephrologist, then built a practice around root-cause testing: inflammation, toxic load, gut and immune markers, nutrient status, and the medication list itself. Conventional care continues. Your existing nephrologist or primary care doctor stays in the loop. What changes is that someone is finally asking why.
Patients come to the Westchase clinic from Katy, Sugar Land, the Energy Corridor, Memorial, Bellaire, Cypress, Pearland, The Woodlands and across Greater Houston.
The clinic is on Richmond Avenue just inside Beltway 8, on the west side of Houston. Parking is on site. Appointments are scheduled, not walk-in, so the time is yours.
Some of what we offer has to be done in a room: EBOO, IV chelation and hyperbaric oxygen. Everything else, including follow-ups, works just as well by video.
Every one of these has causes underneath it. That is where the work starts.
A falling GFR is a result, not a diagnosis. We work out what has been driving it and how much of the decline can still be slowed or moved.
Diabetes is the biggest driver of kidney disease in Houston. Blood sugar is one lever. Inflammation, medications, weight and blood pressure are the others, and they all get looked at.
Pressure that stays high on three or more medications usually has a reason. Kidney and adrenal causes are the first things a nephrologist rules in or out.
PKD runs in families, but how fast it progresses varies a lot. Pressure control, hydration, diet and the right timing for newer medications all change the curve.
IgA nephropathy, FSGS and membranous disease need a nephrologist who reads the biopsy and the immune picture together, and who knows when conventional immunotherapy is the right call.
Lupus that has reached the kidneys is managed with your rheumatologist, not instead of them. We add the part that looks at triggers, gut and immune load.
Low or high potassium, sodium, magnesium and persistent acidosis are kidney questions. They rarely get a proper work-up in a fifteen-minute visit.
After a hospital stay, an infection or a reaction to a medication, the kidneys can keep recovering for months if they are protected properly during that window.
If you have been told dialysis is next, a second opinion is reasonable and common. We look at what has and has not been tried, honestly, and tell you what is realistic.
Functional medicine gets a lot of loose claims attached to it. This is the conventional training underneath it.
Internal Medicine and Nephrology, both through the American Board of Internal Medicine, plus Lifestyle Medicine (ABLM).
Nephrology fellowship at Albert Einstein College of Medicine in New York, internal medicine residency at the University of Illinois.
Former assistant professor at Cleveland Clinic Lerner College of Medicine.
Trained through the Institute for Functional Medicine after her own rheumatoid arthritis diagnosis, and built the HEAL protocol around what she found.
The clinic runs on Central time. Here is what happens between the first call and your plan.
A short form or a call. We ask about your diagnosis, your latest labs and what you want to change. If your case is not a fit for this kind of care, we say so here rather than later.
Labs, imaging, biopsy reports and your medication list. Dr. Bismah reads them before you walk in, so the visit is spent on decisions, not on retelling the history.
An unhurried appointment with Dr. Bismah Irfan. What started the damage, what is still driving it, and which parts of the current plan protect the kidneys versus quietly tax them.
Standard kidney labs plus the tests that usually get skipped: inflammation, toxic load, gut and immune markers, nutrient status. Draws are done in Houston or at a lab near you.
A written plan covering food, medications, supplements and in-clinic therapies where they make sense. Follow-ups are in person or by video, whichever fits your life.
“You are a nephrologist and a functional medicine doctor, which we had been looking for. We did not find one that combines both.”

“When I started with you, my labs were pretty bad. This year I did them again, and everything came back normal.”

No. You can book directly with Kidney Holistic Institute without a referral from a primary care doctor. If you plan to use insurance for labs or imaging, we will tell you what paperwork your plan may want, but the visit itself does not require a referral.
Earlier than most people are told. If your eGFR is under 60, if there is protein or albumin in your urine, if you have diabetes or high blood pressure with any change in kidney numbers, or if your function has dropped noticeably between two tests, that is the time. Stage 3 is where a lot of damage is still movable. Stage 4 and 5 are harder, and still worth doing.
They are the same thing. A nephrologist is a physician who completed internal medicine training and then a fellowship in kidney disease. A urologist is different: a surgeon who deals with the urinary tract, stones and prostate. Dr. Bismah Irfan is a board certified nephrologist.
The training is the same. The difference is what the appointment is spent on. Conventional nephrology manages the numbers and slows the decline. Holistic or functional nephrology asks what started the damage, tests for it, and works on those causes alongside the standard care. Nothing in your medication plan changes without your physicians in the loop.
There is no honest single answer. Houston has excellent nephrologists inside the Texas Medical Center and in private practice. What you can compare is training, how much time you get, and whether the doctor looks for causes or only manages results. Dr. Bismah Irfan is board certified in internal medicine and nephrology, taught nephrology at Cleveland Clinic Lerner College of Medicine, and built her practice around the causes. Read the training history and decide for yourself.
