Work with Dr. Bismah Irfan, MD

Working with me directly starts with this page, not with a calendar.

Tell me where you actually are. I read every application myself and come back to you with the path that fits — honestly, including the ones that cost less.

Most specialist websites give you a booking button. I'm asking you to fill in an application instead, and that deserves an explanation.

One-to-one work with me is the deepest thing we offer, and it is not the right starting point for everyone. Some people need it. Many people get everything they came for from a guided program at a fraction of the price, and putting them on my calendar first would cost them money and time they didn't need to spend. The only way to tell the difference is to know your story before anyone books anything. That's what the questions below are for.

  • Board-certified nephrologist
  • Board-certified in internal medicine
  • Trained and practiced inside conventional nephrology, then left the manage-and-medicate model

You've probably heard some version of this.

"Your kidney levels are off. Something's wrong. Drink water, come back in two weeks and we'll recheck them."
"Let's just keep monitoring."
"He would always just say, you know, you better get ready for dialysis. Here's a piece of paper about high phosphorus foods to stay away from."

Real sentences, quoted word-for-word from consultations with patients who came to us afterwards. Nothing identifying, ever.

People who decided to stop waiting.

What working with me directly looks like.

  • Your full history, read by me personally before we ever speak — labs, medications, what you've tried, and what happened when you tried it.
  • A root-cause map of your case: which of the six drivers of kidney decline are active in you, in what order, written down.
  • A written, ordered protocol — what we address first, what waits, and why. Not a supplement list.
  • My honest answer about what I can and cannot help with in your case, before you commit to anything further.

What we need to know about you

Short answers are fine. Honest answers matter more than complete ones.

An approximate number is fine. A photo of the lab report is even better.

Attach your lab report (optional):

This changes what we'd do first.

Say it plainly. This is the question people skip, and it's the one that tells us the most.

What has never been explained to you properly?

There's no right answer. We're asking because we want to know how you think.

If you're on dialysis, at stage five, or you've had a transplant — please still fill this in. Dr. Irfan will read your history personally and tell you honestly what she can and can't help with. What we won't do is put you into a group program.

The moment you press send, your answers go to Dr. Irfan's desk. She reads them herself and decides the path she'd recommend for you — then we come back to you by email, personally.

Your application

It's in. Dr. Irfan reads it from here.

What happens next

Your application is on Dr. Irfan's desk. She reads what you wrote — not a summary of it — and decides the path she'd recommend in your case. You'll hear from us by email, written by a person, not an autoresponder.

If anything changes in the meantime, or you want to add a lab report you didn't have to hand, just reply to that email or call (281) 394-0238.
